I hereby grant the Greek Orthodox Cathedral of the Annunciation the royalty-free, perpetual right and license for Live Streaming and Social Media for those forms of media specifically consented to above. I understand that this license is granted without expectation of payment or other consideration to me or my child. I certify that I have read this Media Consent and Release and fully understand its terms and conditions.

I hereby release, discharge and hold harmless Greek Orthodox Cathedral of the Annunciation, and each of their respective officers, employees, representatives, agents, volunteers, successors and assigns from any and all claims, demands, and liabilities arising out of or in connection with the creation and use of the aforesaid materials as indicated above.
